Two S2S documents give different street addresses and fax numbers for this agency. The Round-2 tracker says 3045 Sacramento St., Unit/PO Box 647, Placerville, CA 95667 with Fax (909) 366-3224; the “Sliding Scale and Procedures” letterhead (headed “Standard prices as of 9/30/2024”) says 3987 Missouri Flat Rd. Suite 340-332, Placerville, CA 95667 with Fax (909) 752-5445. City, ZIP, phone and website match in both. We render the Round-2 values, and both are now corroborated: S2S’s own Financial Assessment Form — a live client-facing document — carries the same Sacramento St. address and the same fax on its letterhead, and the compliance email templates carry the fax as well. That is three current S2S sources against one 2024 sheet. What is still missing is a word from S2S that they moved. S2S: can we treat the “Sliding Scale and Procedures” letterhead as out of date and due for reprinting?
